• Purchase Agreement
• Home address for the past two years
• Home phone number
• Name, address and phone number of current employer(s)
• Names, addresses and phone numbers for all employers, last two years
• Accurate gross income information - current pay stubs
• Last two years tax returns
• Addresses, account numbers and current balances for all checking, savings and other deposit accounts
• Copy of last two bank statements
• Addresses, account numbers, current unpaid balances and monthly payment amounts for all debts
• Information about all real estate owned and existing mortgages
• Social Security and/or other benefit award letters
• Self employed or commissioned:  tax returns for previous two years/YTD profit and loss statement
